• Proof of applicant’s U.S. citizenship: Copy of U.S. passport or certificate of naturalization
• Applicant's 3-year tax return form I-864

• Copy of parents' marriage certificate
  
• No criminal record of the father/mother
• Birth certificate of both parents
• Copy of the passport of the parent
• Copy of the parent's non-immigrant visa (such as B visa)
